• Data Line Number (if data plan for tablet)

Additional Information

If a unit has a cell phone and or device that is no longer needed within the unit a Declaration of Surplus Form should be completed and emailed to Property Disposition – unit will be given instructions on the next step after the form has been received.

Personal cellular phone services are not part of this contract.
